Dr. Arun Rathi is a Visiting Consultant Urologist and Andrologist at Vivekananda Hospital, Begumpet, with over 18 years of clinical experience. Routine urological procedures including stone surgery (URS, PCNL, ESWL), TURP, hydrocele and varicocele surgery are performed at Vivekananda Hospital. What is the difference between MCh Urology and a general surgeon who does urology?
MCh Genitourinary Surgery is a 3-year super-specialty after MS General Surgery. It is the formal urology qualification recognised by the National Medical Commission. General surgeons who do occasional urology cases lack this structured specialty training. For complex urology, the MCh qualification represents a meaningful difference in depth of training.
I have a small kidney stone. Do I need surgery?
Most stones under 5 mm pass naturally with increased fluid intake, medication to relax the ureter, and pain management. Stones between 5-10 mm may pass with patience or may need intervention depending on location and symptoms. Stones above 10 mm typically need active removal (URS or PCNL). Dr. Rathi reviews imaging and symptoms before recommending a specific approach.
What is PCNL for kidney stones?
Percutaneous Nephrolithotomy (PCNL) is a keyhole surgery for large kidney stones (above 2 cm). A small incision is made on the back, a tube is passed into the kidney, and the stone is broken and removed. PCNL is more effective than other techniques for large or complex stones, with hospital stay of 2-3 days. Recovery to routine activity is typically within 2 weeks.
I am an older man with difficulty passing urine. Is this just age?
Urinary symptoms in older men are commonly due to BPH (benign prostate enlargement), which is age-related but treatable. Symptoms include weak stream, starting delay, frequent night-time urination and incomplete emptying. Evaluation includes PSA blood test, ultrasound and uroflowmetry. Most cases respond to medication; TURP surgery is reserved for medication failure or complications. Not every urinary symptom is BPH - evaluation matters to rule out infection, stones or prostate cancer.
My wife and I are trying to conceive. Can a urologist help with male factor?
Yes. Male factor contributes to about 40-50% of infertility cases. Evaluation starts with semen analysis and medical history. Common treatable causes include varicocele, low testosterone, infection, obstruction, and lifestyle factors (smoking, heat exposure, stress). Treatment ranges from lifestyle modification and medication to varicocele surgery and fertility-preservation techniques. What is varicocele and when does it need surgery?
Varicocele is dilation of veins in the scrotum, found in about 15% of men. Most are asymptomatic and need no treatment. Surgery is indicated when varicocele causes pain, testicular atrophy (shrinkage), or male infertility. Varicocelectomy is a day-care surgery with good success rates for pain relief and semen parameter improvement. Not every varicocele on ultrasound needs surgery.
I have blood in urine but no pain. How serious is this?
Painless blood in urine (haematuria) always deserves urology evaluation, regardless of age. Causes range from harmless (urinary infection in certain patterns) to serious (bladder or kidney cancer). Evaluation includes urine tests, imaging (ultrasound or CT scan) and cystoscopy if needed. The appearance of pain-free blood matters less than the fact that it occurred - investigation rules out malignancy early when treatment outcomes are better.
Are urology procedures covered by insurance?
